FINANCIAL POLICY
For the convenience of all of our patients, we accept cash, personal check, Care Credit, MasterCard, VISA, & American Express. Every effort is made to calculate your patient portions for each date of service. However, they are still only estimates based on the current information we have. The exact amounts are not factually known until the claim has been paid and received. We file contracted insurance as a courtesy; however, you are responsible for any balance amount remaining after 30 days.
- A deposit is required in order to reserve an appointment. The deposit is equal to ½ of the estimated patient portion for that appointment and is non-refundable if you should fail to give 2 business days’ notice of appointment cancellation.
- All procedures are charged and due at the time of service. If you have insurance we may file it as a courtesy to you. All estimated patient portions associated are due at the time of service. All remaining balances are due after 30 days regardless of insurance.
- Dental coverage is between you, your employer, and the insurance company. It is your responsibility to understand what services are covered, if you are in or out-of-network, or if you have deductibles, etc. If the estimated insurance payment is not received within 30 days from date of service, you will be expected to pay in full.
- Returned Checks / Declined drafts will be charged a non-refundable $50.00 fee for each occurrence.
- Balances not paid in full after 60 days will be subject to collections. We may instill the help of an outsourced collection agency, and/or small claims court. Once an account balance extends past 90 days, a 33% collection fee will be added to the account, plus any court fees or attorney fees associated with the pursuit of collections. Furthermore, the patient(s) on the account will be dismissed from the practice.
- We will not allow new appointments to be scheduled for any patient under an account with a past due balance.
MISSED APPOINTMENT & TARDY POLICY
When you book an appointment with us, we reserve a block of time specifically for you, with our team members and equipment prepared for you. We know, however, that emergencies do occur and will work with you when possible. However, in order to be fair to all patients, we find it necessary to have an appointment policy enforced.
- Arriving 10+ minutes late for appointments is considered tardy & may require rescheduling.
- Failure to give 48 hours (2 business days) notice is considered a missed/ cancelled appt.
- First offense: Courtesy reminder of our office policy.
- Second offense: You will be required to pay a non-refundable deposit before scheduling your next appointment(s). The deposit is equal to $50 or up to 50% of the expected patient portion for the appointment. If the appointment is missed, a missed appointment fee of $50 or up to 50% of the expected patient portion is charged to the account. If appointment is kept the deposit remains on account for future scheduling.
- Third missed appointment may result in office dismissal.
APPOINTMENT CONFIRMATION POLICY
- You, as the patient or guardian, are responsible for providing our office with a reliable contact information regarding appointments.
- If we are unable to obtain confirmation regarding your scheduled appointments, we reserve the right to cancel your appointment.
